Fill in the Form Please fill in our internet form. This is initial direct contact with us. The form contains contact details, some general information about you and the treatment you are interested in.

Medical and Stay Questionnaire
When we receive internet form, we will send you more precise medical questionnaire to complete. Remember to fill it up openly and precisely. In addition, we will ask you about your plans for leisure time in Poland and your expectations for accommodation. After obtaining from you all needed infromation ( may happen that we will ask for additional data) we will be able to give you an offer.

Offer and Acceptance You will receive our offer for treatment (including different options), accommodation, tourist attraction in free time, spa treatment (if wanted). When you accept our offer, we will be asking you to pay in advance some amount of money, of total treatment cost/accommodation price, to secure whole operation (project).

Airport Pick Up, Hotel Check In, Payment
St. Catherine Assistant will take you from the airport to your hotel for check in. After check in, we will ask you for payment of remaining costs. Same day or day after, you will be taken to clinic to see your personal doctor, discuss all circumstances, do some tests, preparing. Your time will be divided to your visits at your doctor’s office and your free time/leisure. It all depends on your treatment/spa how much time is left for you, we can always help with organizing your free time. |

Treatment/Spa This step looks differently, depends what kind of goal you have to achieve- relax or treatment. After treatment your doctor will present you all precautions and some MUST DO in your recovery time. In case of only wellness stay you will be taken to your spa / wellness hotel.

Evaluation Form It is very important for us to provide possible best service, that is why, we will be asking you to fill in our evaluation form, when you can share with us your opinions, comments, complaints about St. Catherine Travel Agency and your stay in Wroclaw. |

Goodbye/Airport Your Personal Assistant will drive you to Wroclaw's airport and you can go safely back home on yours “new wings”.
